Refer a Patient

Physicians can seamlessly refer patients using MyChart by electronically submitting a referral through their electronic health record (EHR) system. Once submitted, the patient is automatically notified and prompted to schedule the appointment via a secure link in the message or directly within their MyChart portal.

Referral Process

1. Initiate Referral The physician creates a referral or order within their EHR system. This functions similarly to completing a referral form in external platforms like ChartRequest.

2. Patient Notification Upon submission, the patient receives a real-time alert via text, email, or push notification. This message includes a direct link guiding them to schedule the appointment.

3. Appointment Scheduling The patient logs into MyChart, follows the scheduling prompts, provides any required information, and confirms the appointment.

4. Record Integration & Access The referral is automatically integrated into the patient’s medical record. The receiving specialist can then access relevant clinical history and documentation through the shared network, ensuring continuity of care and preparation for the visit.
