You may fax, mail, or personally deliver your completed form to the Health Information Management Department. We will need a valid photo ID (driver’s license, military ID or state ID) and a telephone number.

All hospital medical records release requests for adults and children are processed downtown on the OU Health Center campus in the Oklahoma Children's Clinics Building. Per law, we have 30 days to process and release medical records; however, our goal is within 7 days. Use the information below to submit a request or inquire on status.
