Medical Records Request Forms

• Request for Health Information/Treatment Records (For Self/Child's Records or Attorney)

• Authorization to Release Health Information/Treatment Records

You may fax, mail, email or personally deliver your completed form to the Medical Records Department. We will need a valid photo ID (driver’s license, military ID or state ID) and a telephone number.

Please call (918) 619-4491 if you ​have any questions about you or your family’s medical records. Allow five to seven days to process your request.
