To obtain copies of your health information and medical records, you have the option to access your records online via OU Health’s patient portals.

If you’d like to pick up Oklahoma City metro- area hospital, clinic and ER records in person, you can download, print and complete the form below. Please check as many items needed and be sure to include encounter dates.

You may fax, mail, or personally deliver your completed form to the Health Information Management Department. We will need a valid photo ID (driver’s license, military ID or state ID) and a telephone number.

All hospital medical records release requests for adults and children are processed downtown on the OU Health Center campus in the Oklahoma Children's Hospital Clinics Building. Per law, we have 30 days to process and release medical records; however, our goal is within 7 days. Use the information below to submit a request or inquire on status.
