Referring Physicians - Children's Genetics

Thank you for your referral. Please download the OU Children's Physicians Referral Form, fill it out and fax it in to (405) 271-1154. Please include the following information so that the appropriate medical provider is selected for your patient.

In order to process the appointment, please include the following information.

• Legible copy of Insurance card(s) (front and back)
• Office note pertaining to this referral
• If required, copy of referral or authorization
• Lab results pertaining to this referral

Please print out the following documents and give them to your patient before they depart your office:

• Review of Patient
• Questionnaire
• Questionnaire (Spanish)
• Map and Directions
