Referring Providers

Thank you for your referral and for choosing the Children’s Heart Center to provide subspecialty care for your patient.

To refer a patient to one of our Children's Heart Center specialists, please contact the following:

• For referrals to Pediatric Cardiology, please call (405) 271-5530 for assistance. We encourage you to download and fill out this referral form. Fax the completed form and required support documents listed below to (405) 271-2034.
• For referrals to Pediatric Cardiothoracic Surgery, please call (405) 271-4631 for assistance. We encourage you to download and fill out this referral form. Fax the completed form and required support documents listed below to (405) 271-5190.
• For requests to obtain Pediatric Echo Lab services, please call (405) 271-5918 for assistance. We encourage you to download and fill out this request form. Fax the completed form and required support documents to (405) 271-1162.

In order to expedite the processing of the appointment, please include the following information:

• Face-sheet with patient demographics, including patient, next-of-kin, guarantor and insurance information
• Referral form verifying eligibility, period of coverage and reason for subspecialty care (if required by payer)
• Most recent clinic note(s) and pertinent diagnostic results (e.g., EKG, Lab, X-Ray, etc.)
• Legible copy of both sides of the patient’s insurance card

Note: Pre-authorization/certification for services will be requested and obtained by the Children’s Heart Center support team.
