Experts at the Child Study Center provide services to children, adolescents and their caregivers in the assessment, diagnosis and treatment of ADHD, autism, drugs and/or alcohol exposure before birth, behavioral and/or emotional difficulties and trauma exposure.

• Referral Packet – To request services at the Child Study Center
• New Patient Questionnaire – Once your referral has been processed, return this completed form to the Child Study Center before your next appointment
• Review of Systems Interval History – For all new and returning developmental and behavioral pediatrics (DBP) medical patients
• Vanderbilt Parent Diagnostic – For all new patients being evaluated for attention deficit hyperactivity disorder (ADHD)
• Vanderbilt Parent Diagnostic Español – For all new Spanish-speaking patients being evaluated for ADHD
• Vanderbilt Teacher Diagnostic – For teachers of all new patients being evaluated for ADHD
• Vanderbilt Parent Follow-Up – For all returning patients with ADHD
• Vanderbilt Parent Follow-Up Español – For all returning Spanish-speaking patients with ADHD
• Vanderbilt Teacher Follow-Up – For teachers of all returning patients with ADHD
• Vanderbilt Teacher Follow-Up Español – For teachers of all returning Spanish-speaking patients with ADHD
