Community Health Worker & Representative Training

Dementia Training for Community Health Workers and Community Health Representatives was developed a part of the OkDCN. The goal of this training is to deliver dementia care education and skills to CHW’s and CHR’s across the country. The training is evidenced based and includes basic information on dementia and how to address dementia based on the C3 core CHW roles.

The training is delivered virtually and has two models available. CHW/CHR Peer-led Dementia Training and Train-the-Trainer Training.

Dementia Training for Community Health Workers in Oklahoma

Successful completion of both parts is required to obtain the training certificate.
