Details:

The Oklahoma Dementia Care Network invites CHWs/CHRs at your organization to participate in our upcoming Dementia Training for Community Health Workers (CHWs) and Community Health Representatives (CHRs).

This training will be offered in a hybrid format:

• Self-paced online module (1 hour) — to be completed by March 20
• Live Zoom session — Wednesday, March 25, 9:00am–2:00pm CST
• Total training time: 6 hours

Training Purpose

• Provide dementia training tailored to CHWs/CHRs
• Offer a CHW/CHR training program
• Issue dementia-specific certificates for CHWs/CHRs

Training Content

• Basic information on dementia
• CHW C3 10 Roles in Addressing Dementia
• Resources for CHWs/CHRs
• Training evaluation materials

Participants will complete a brief knowledge survey before and after the basic module, as well as a feedback survey following the live session. A Certificate of Completion will be provided. Registration
