Each of our Age-Friendly ECHO sessions are devoted to case presentations by a community support professional team member, followed by a 25-minute informational presentation delivered by a guest speaker and a 15–minute QI by the ECHO Hub Team. We will focus on the 4Ms of age-friendly care: What Matters Most, Medications, Mentation and Mobility. When implemented together, the 4Ms represent a commitment to focus on the unique needs of older adults. The ECHO Hub Team will also provide community-based resources to improve the care of older adults.

OkDCN ECHO Special Session series is designed for all participants of ECHO and any healthcare providers or service professional who wishes to learn more about Age-Friendly care. These 60-90 minute sessions are scheduled throughout the year to provide additional information to professional serving individuals living with dementia as well as their family caregiver.
