Age-Friendly Health System Resources

Age-friendly care is an important part of providing high-quality and comprehensive care within a health system. However, there are clear definitions on what it means to be age-friendly and what it means to be recognized as an official Age-Friendly Health System.

Age-Friendly Health Systems

To officially be recognized as an Age-Friendly Health System, institutions must demonstrate their ability to reliably deliver care to all older adults in their system using a set of four evidence-based elements of high-quality care, known as the "4Ms." The 4Ms include: What Matters, Medication, Mentation and Mobility. When implemented together, the 4Ms represent a broad shift by health systems to focus on the needs of older adults.

What does it mean to be age-friendly?

  • Follows an essential set of evidence-based practices
  • Causes no harm
  • Aligns with what matters to the older adult and their family or other caregivers

What Is The Age-Friendly Certification?

  • Level I (Participant) teams have successfully developed plans to implement the 4Ms
  • Level II (Committed to Care Excellence) teams have three months of verified data to demonstrate early impact of using the 4Ms
